Job Description

Position Overview:

As a Social Worker at Faith Charitable Trust's SUD Center in Bhubaneswar, you will be an integral part of a multidisciplinary team dedicated to providing comprehensive support and assistance to individuals struggling with substance use disorders. You will work closely with clients, their families, and community resources to facilitate access to treatment, promote recovery, and address underlying social issues contributing to substance abuse.

Key Responsibilities:

Client Assessment and Support:

Conduct initial assessments to evaluate clients' needs, strengths, and barriers to recovery.

Develop individualized care plans in collaboration with clients and other team members.

Provide counseling, crisis intervention, and emotional support to clients and their families.

Case Management:

Coordinate referrals to medical, psychiatric, and social services as needed.

Advocate for clients' rights and access to appropriate services within the community.

Monitor clients' progress and adjust treatment plans accordingly.

Community Outreach and Education:

Develop and maintain partnerships with local organizations, healthcare providers, and government agencies to enhance services for individuals with substance use disorders.

Conduct educational workshops and presentations on substance abuse prevention, harm reduction, and treatment options.

Raise awareness about substance use disorders and reduce stigma through community engagement initiatives.

Documentation and Reporting:

Maintain accurate and confidential client records in compliance with organizational policies and legal requirements.

Prepare reports, progress notes, and other documentation as needed for funding agencies, accreditation bodies, and internal purposes.

Team Collaboration:

Collaborate effectively with other members of the interdisciplinary team, including counselors, psychologists, medical professionals, and administrative staff.

Participate in team meetings, case conferences, and professional development activities to ensure coordinated and high-quality care.

Qualifications:

Master's degree in Social Work or a related field.

Previous experience working with individuals with substance use disorders or in a related mental health setting is preferred.

Knowledge of community resources, social services, and referral networks.

Strong communication, interpersonal, and advocacy skills.

Ability to work effectively both independently and as part of a team.

Sensitivity to cultural diversity and a commitment to promoting social justice and equity.

Additional Requirements:

Valid driver's license and access to reliable transportation.

Willingness to work flexible hours, including evenings and weekends as needed.

Adherence to ethical standards and professional boundaries in all interactions with clients and colleagues.
