About the project:

Swasthya Sankalp is committed to improving public health outcomes, with a particular focus on Non-Communicable Diseases (NCDs) such as hypertension and diabetes. As part of our efforts to enhance comprehensive primary care services, we are seeking a qualified and experienced individual to serve as the State Program Officer for the state. The primary objective of this position is to provide technical and programmatic support in the design, implementation and monitoring of cardiovascular health interventions, focused on improving control of hypertension and diabetes at the primary health care level in selected geographies in the state. The SPM will work closely with the State NCD Nodal Officer and Comprehensive Primary Health Care (CPHC) Nodal Officer to implement strategies for improving screening, diagnosis, treatment, and follow-up care for individuals with hypertension and diabetes at the primary health care level.

Job Description

Expected role of the incumbent is multi-dimensional and are summarised below: 

Conduct landscaping and situational assessment of hypertension and diabetes service delivery at primary healthcare level across implementation geographies.

Collaborate with the State officials to develop and implement state-specific strategies for strengthening hypertension and diabetes management and control at primary health level.

Provide technical assistance and strengthen the capacity of the State and selected districts for planning, training, supervision and monitoring of hypertension and diabetes service delivery.

Work with state and district officials for developing and implementing state program implementation plans (PIP) towards ensuring adequate financial resources under NP-NCD and CPHC program.

Strengthen opportunistic screening at the health facilities and streamline linkages with population-based screening for diagnosis of hypertension and diabetes.

Accelerate implementation of hypertension & diabetes protocol-based patient-centred services.

Assist in planning and monitoring uninterrupted availability of anti-hypertensive and anti-diabetic drugs at all AAMs in intervention geographies and strengthening of overall drug procurement and supply chain system.

Identify knowledge gaps and opportunities for improvement in knowledge management processes and propose and implement solutions to address them.

Conduct needs assessments and gap analyses to identify areas for improvement in chronic disease management services and develop strategies to address them.

Collaborate with internal and external stakeholders to promote awareness of NCDs, risk factors, and preventive measures through community outreach programs, workshops, and educational materials.

Conduct regular supportive supervision visits to implementation sites and provide technical assistance and guidance to AAM staff to ensure adherence to evidence-based guidelines and best practices in hypertension and diabetes management.

Support uptake and utilisation of NCD information system, monitor key indicators, and provide regular reports to relevant stakeholders to improve program implementation.

Participate in meetings at block, district and state level and provide timely updates/ reports to health department.

Support the health department in finalising PIP for NCD at district / state level.

Map/identify non-governmental agencies actively involved in NCDs in the identified district/s and develop networks. 

Streamline institutional mechanisms for regular review of NP-NCD program and support rapid cycle process improvements.

Prepare monthly reports on the progress of the project with follow-up on action points and share feedback to state NCD and CPHC cell.

Support state officials in making technical presentations, strategic documents required for decision making and documentation of good practices as relevant to the project.

Any other task assigned by appropriate authority. 

The position requires extensive travel (up to about 10 days in a month) within state. 

In addition to the above-mentioned roles and responsibilities, s/he will collaborate with existing projects initiatives in the state to integrate hypertension and diabetes management into performance monitoring frameworks for CPHC services.

Qualifications & Experience

The SPM should possess the following qualifications and experience: 

Medical/Nursing graduation with an essential post-graduate degree in public health/Health Administration/Health Policy, or a related field is required. A specialization or coursework related to chronic diseases like hypertension, and diabetes is preferred.

At least 5+ years of relevant experience in public health program management, preferably in the field of non-communicable diseases (NCDs) with a focus on hypertension, diabetes and health system strengthening.

Experience in working with government health agencies at state level.

Strong understanding of primary healthcare systems, including experience working with public health facilities at the state or district level.

Experience in designing, implementing, and monitoring health programs, particularly those targeting prevention, early detection.

Demonstrated experience in working with government health agencies, NGOs, and other stakeholders involved in healthcare delivery.

Experience in capacity building, training, and providing technical assistance to healthcare providers and community health workers.

Proven track record of managing budgets, procurement, and financial reporting related to health programs.

Excellent analytical, communication, and interpersonal skills.

Proficiency in MS Office package.

Fluent in English, Hindi, and the proposed state’s language, enabling effective communication with stakeholders at all levels within the state. 

Note: -

Incumbent is expected to have excellent writing skills and language proficiency, for hich a screening test may be administered.
